Instructions Here are the simple steps for submitting your Pet Memorial by mail: 1.  Click here to download a PDF version of the Memorial Submission form you can complete and mail to the P.E.I. Humane Society.   2.  Click here to get instruction for an online sumbission. 3.  Return the completed form, photo of your pet to the P.E.I. Humane Society at: PO Box 20022, Charlottetown, PE  C1A 9E3. There is no fee to submit a Pet Memorial however, should you wish to make a donation to the P.E.I. Humane Society in memory of your pet, you may mail your cheque or money order, payable to the P.E.I. Humane Society to Box 20022, Charlottetown, PE  C1A 9E3. All donations of $20.00 or more will receive an official tax receipt.  4.  Your Pet Memorial will be posted to its own unique webpage on the P.E.I. Humane Society website within three business days after receipt
